CARE4CKD is an Erasmus+ project with the goal of raising awareness and promoting understanding of Conservative Care as a holistic, patient-centered approach for individuals with Chronic Kidney Disease (CKD)—especially elderly or frail patients for whom dialysis or transplantation may not be suitable.
