The project adopts a collaborative and patient-centered approach, connecting nephrology centers, patient associations, and universities across Europe to promote conservative CKD care. Through focus groups, webinars, and workshops, partners will share experiences and co-create educational materials. The joint work will result in a Good Practice Guide and diverse training resources, including videos, e-learning modules, and case studies. Continuous evaluation through surveys and meetings will ensure quality and assess impact, while dissemination via conferences, digital platforms, and open-access materials will support sustainability and the transfer of good practices across Europe.
